Dear Sir, I woke today’s morning with a severe headache. After breakfast, I took a painkiller thinking it would get better as usual, but I did not find any relief and it is worsening. I want to let it check by visiting the doctor’s clinic and need leave in this regard. I will be highly obliged to you for your kindness. I promise after coming back I will catch up on the workload.

Hope you have been doing well. I am writing this for leave from the office as I have chronic migraine. it is killing me and has diverted my attention to focus on work. I usually keep my medicine with me but unfortunately, I have forgotten it at home today. The doctor’s clinic is near the office, so I want to take an appointment with a doctor to get myself checked. I shall be very thankful to you.

Respected Madam, I am having a headache since last night from the office to home. I think that it is because of heatstroke that is getting to my head and even at home I am unable to focus on anything. Hence, it would not be possible for me to come to the office today as I would not be able to do any kind of work. I know that our clients put much pressure on us but I have talked to Albert Stalin and he will handle my workload today. I make sure that tomorrow I will reach the office in time and will work extra hours as compensation for today’s leave. Kindly allow me today’s leave.

Dear Sir, I have always been dutiful and professional at the workplace and for this, you call me the best employee. You also know that I always keep my headache medicine with me as I get headaches on daily basis. I need to refill the medicine box as my antibiotics have finished. I have to refill it today and I have no one at home that can refill it on my behalf, so I have to go myself to pick it up from the pharmacy. In this way, I will be able to intake medicine to find some relief from pain. If you could give me permission to leave early, I shall be very thankful to you.

I hope you have been doing well. It is not possible for me to pay full attention to work right now as I am having chronic migraine. If I continue to work, then there is the possibility of my getting faint. I need to consult a doctor immediately in this regard. If you could allow me to leave early so may I get myself checked timely? I shall be very thankful to you for this act of kindness.

Please take this message as a request to leave early today as I am having a headache now. I had a lot of work pressure on me for the past ten days and it had made me very low and lethargic. Right now, I am not feeling good, and I have this fear to get fainting soon. I would be highly obliged to you if you could leave me early today.

I am writing this to let you know about my leave today from the office due to chronic migraine. I was trying to give my best for the past few days, but I became unable to do so because of a seething headache. I almost fell down yesterday due to a continuous headache and have taken medicine for it but it seems to be of no help. I am afraid that it will not be possible for me to mark to the office today as I have to visit my doctor to investigate the matter. I hope that it will be cured timely, and I will be able to come to the office tomorrow. I am sorry in advance for the inconvenience it may cause you due to my absence. I will try to make up for it after I return.
